Outreach Teams and Regional Coordinators

The Acquired Brain Injury (ABI) Outreach Teams and Regional Coordinators work to bridge the gap in service between acute care/rehabilitation and the community. They assist the survivor through the reintegration process focusing on quality of life for the survivor and the family.

A variety of services may be offered including: coordination of community resources, family support and education, and identifying and supporting appropriate residential options. In order to implement individualized care plans, community development is a central role. The Outreach Teams and Regional Coordinators also facilitate education and training of other human service providers.
