The Acquired Brain Injury (ABI) Outreach Teams and Regional Coordinators are part of the ABI Partnership Project, a community-based service continuum managed by Saskatchewan Health and funded by Saskatchewan Government Insurance (SGI). These services work to bridge the gap in service between acute care/rehabilitation and the community. They assist the survivor through the reintegration process while focusing ultimately on an optimal quality of life for the survivor and the family. The Outreach Team and Regional Coordinators’ functions fall within three main areas: identifying, coordinating and supporting community programs and resources that meet the individual needs of the ABI survivor; family support and education; and identifying and supporting appropriate residential options. In order to implement individualized care plans, community development is a central role of the Outreach Teams and Regional Coordinators. These services also facilitate education and training of other human service providers.

Each Outreach Team consists of a variety of rehabilitation professionals experienced in the field of acquired brain injury. The Outreach Teams are responsible for providing services in the following health regions:

Sask North (located in Prince Albert)
Prince Albert Parkland, Kelsey Trail, Athabasca, Keewatin Yatthé, Mamawetan Churchill River Health Regions

Sask Central (located in Saskatoon)
Saskatoon, Heartland, Prairie North Health Regions

Sask South (located in Regina)
Regina Qu'Appelle, Sun Country, Five Hills, Cypress, Sunrise Health Regions

For more information, please contact the team responsible for your health region. The teams accept self-referrals as well as those from health facilities, physicians, any rehabilitation program or professional support services, schools or community agencies.

ABI Regional Coordinators


There are five ABI Regional Coordinators located throughout the province.  They are located in the communities of Swift Current, North Battleford, Weyburn, Yorkton and Moose Jaw and serve their respective health regions. The Coordinators engage in community development and case management activities to facilitate successful community reintegration of individuals with ABI.
